SYP response:

With the exception of Warrant Cards and Radios, for the most part we do not record whether an item has been lost or stolen.

They are often recorded as having been issued, and this can apply to a variety of instances- i.e. routine replacement or issuing to a new recruit. However, for the most part, information regarding the reasons behind issuing a new item of equipment is not recorded.
