1. Details on the number of times since August 2004, the Force (see definition in point 4) has been unable to provide, in full, exhibits, evidence, case files or notes and/or any other such materials as requested by a legitimate requesting authority be that an individual or any other such body including, but not limited to, the Criminal Cases Review Commission, the Independent Police Complaints Commission, solicitors working on appeals against convictions, the families of victims and any other campaigning organisations?
 
2. The number of instances, covering the same dates, where requests to the Force to provide in full any exhibits, evidence, case files or notes and/or any other such materials subject to a ‘preservation order' under Section 17 of the Criminal Appeal Act 1995 as issued by the Criminal Cases Review Commission, have not been possible in its entirety due to any or all of the requested materials being lost, misplaced, damaged, destroyed, stolen or otherwise rendered irretrievable?
 
3. Any other instances covering the same dates - irrespective of whether a request for exhibits, evidence, case files or notes and/or any other such materials been made or not - where the Force is aware that any such material has been lost, misplaced, damaged, destroyed, stolen or otherwise rendered irretrievable?
 
4. For the purposes of this FOI request the word ‘Force' defines the Police Force or Constabulary for which the addressee is responsible for dealing with FOI requests.
